Responsibility for failure

We take responsibility for our mistakes and learn how not to do that again. But more importantly we share that learning or better still play a role in a bigger learning exercise, so that nobody else has to make that mistake again.

We do it by examining why we made that mistake: What pressure were we under? What distractions? What poka yoke should there be? …
The key is the psychological safety to share our experience and our ideas, not hide them.

It’s a matter of professionalism that our mistake hurts us. It’s a matter of safety that nobody else does.
